ATHLETIC CLEARANCE PROCESS
Athletic Clearance is required prior to participation in JBHS Athletics & Activities, including tryouts.
1) New Athletes - Go to www.homecampus.com/login. Set up a user account and password (School Year; School: BURROUGHS/BURBANK-SS). Provide all the information and electronically sign all required forms. Be aware that you will need emergency contact information and proof of insurance to complete the process:*
Current/Returning Athletes – To update your account (e.g., upload new Physical Form, add new sports to your list, etc.), log in to your existing account with your created user name and password, select the correct school year and school. Then upload the updated Physical Form.
2) The Official Physical Exam Form must be used. Click on the link above to access the form. It must be completed and signed by the parent, signed and stamped by an MD and uploaded to the Home Campus account. Incomplete forms will be denied until required document is submitted.
*Note: Medical Insurance is required to participate in High School Activities. Any athlete without medical insurance must purchase school insurance at a minimal cost. You may also visit the Aldrich Taylor Insurance website and purchase the insurance online, www.BurbankStudentAccident.com. Please bring verification of purchase to the first day of practice.
- Athletes will be cleared for participation after the required documents have been properly filled out, signed, and uploaded.
- If you do not make a team before school starts,
you may be placed in a 7:30am Physical Education class. - Visit our School website at JBHS.Burbankusd.org.
